Forms and Paperwork Submission
Forms and paperwork can be submitted via the following methods:
- Securely uploaded to the MemberDirect message center
- Submitted through postal mail to the COAERS mailing address:

- Members can securely drop off documents to the COAERS office during normal business hours Monday through Friday, from 8 am to 5 pm
